PLAYER’S FEE: $35.00 for 2011 (payable to PDA), due 1st night of play.

SPONSOR’S FEE: $30 Wednesday and Thursday / $15 Tuesday

GENERAL MEETING: (Winter 2012) and TROPHY PRESENTATION (Fall 2011) will be Thursday January 12, 2012 7:30 PM, location is TBD.

All teams must send a team member to the meeting. League starts January 17, 2012.

To signup, send in a completed application page or take it to The Dart Shop.  You can also email your team information to signup@pacificdarts.com.  Be sure to include Team name, sponsor name, address and phone number, night of play, division desired, Team Captain's name and phone number.

Fill out all required information and then click on submit at the bottom of the Page, items in Red are required.  If sponsor is not listed in menu please type it in.  Tuesday is 2 person teams, Wednesday is 4 person teams, and Thursday is 3 person teams.
